We provide an online application process. But for those who prefer a printable version we have provided the Loan Application as a PDF that you can print at home. This is a legal size document, so do not forget to change your print set-up to 8 1/2 by 14 before you start to print. After completing the loan application, print, sign, and fax them to OnLine Mortgage at (801) 944-4102.
